Review Quiz always shows Incorrect when all questions are correctly answered.

what are the 'revisit' option selected on the quiz slides. I've seen this happen if the revisit option is set to 'reset to intial state' as it removes the answer the user had selected meaning there is no selection and shows the red banner.
