I know you mentioned the course is in the LMS, but I did want to share just in case that Review 360 is a great way to gather feedback (with screenshots) beforehand. But I also know sometimes, things can still go unnoticed. To go along with what Marc shared, perhaps adding a custom tab to the player could do the trick. For example, you could name the tab "Report a bug" and because it's in the player, it would always be visible and clickable when the learner needs it. You can have that tab lightbox a slide with specific instructions, open a URL to report, send an email, or more. Just throwing out some ideas! I hope you find something that works for your needs!
