- Hi Jackie, do you have this available as a download? I'd like to see how you built things. I've got a very similar need in a Health Care Authority with many forms that have to be filled out by a wide variety of people when charting patient/client notes.

I always recommend having control over your own server space so you can upload and share your work. Most designers tend to have their own websites (with paid hosting) and share their portfolios there - which is what I do on mine: http://www.jackievannice.com. I hope that helps!
