Radio button issue

Hey all,

I am learning on the job so to speak and creating a likart scale that is suppose to tally a score on the results page based on which radio buttons the user clicks on in response to each question.  It only allows me to do 5 rows of buttons, for rows 6-10 when I click on the radio button, it will deselect the row before it. I'm not able to figure out what I'm missing here. Any advice is appreciated.  Thank you.
