"You should assume that any image you find online is protected by copyright." To put a fine point on this statement, by default a photo (or written/artistic/musical work) is copyright *upon creation*, regardless of whether or not you see a Ⓒ symbol. The creator can add a Ⓒ symbol, or go through the process of paying for a copyright to legally protect their intellectual property in the event of a challenge.

To be technical, the creator can pay to register their creation, which adds a layer of legal protection. As Karen says, it's already copyrighted upon being created "in a fixed form" (meaning an idea can't be copyrighted. It must be written down, drawn, recorded, etc.)
