In Grad school, one of the best and most useful books I ever had to buy was "Developing and Using Classroom Assessments" by Albert Oosterhof. It really helps to explain what makes a good assessment question. Plenty of examples and summary charts for quick reference. I used to think that K-12 and Corporate Instructional Design were different. They're not. I use almost everything from my public school and University teaching days.

Thanks, Amy. Anyone else have good book-length tips on this assessment issue?
Nicole, I like the brief overview above--well done--just looking for more details.
