I always recommend having control over your own server space so you can upload and share your work. Most designers tend to have their own websites (with paid hosting) and share their portfolios there - which is what I do on mine: http://www.jackievannice.com. I hope that helps!

- I really like the interaction you created here. This is a really good way to do a mini-tutorial for someone who may already be familiar with this form, or something to accompany a training course that explains the form and the various reasons for checking this or that checkbox or field on the form in more detail.
