I like what Ryan did and have an idea for how to create something similar, so time permitting you may see something from me later this week. I just finished a medical professionalism game in which interactive conversation occurs on several levels. First, there is the conversation between the learner and the patient. Typical simulation interaction type of stuff - you see the patient's face, you hear their issue, you answer a question - and then you see the patient's look and hear his (her) voice tone change in response to how you answered their question. That conversation is intrinsic feedback. But I've also built in some extrinsic feedback - in the form of conversation between the learner and an instructor. Both forms of feedback - and therefore both conversations - occur simultaneously.
Now - here's the really cool part! When we tested this initially, we found that after learners (medical students and residents) went through the course the first time, they wanted to go back through it a second time - to see what would happen if they INTENTIONALLY selected all of the WRONG answers. So we had fun, built that it, and called it level 2. It's well worth checking out!

Once you get to your first scenario - and you'll know when you do - look up in the upper left corner for the title "PROFESSIONALISM" Hold your mouse inside the closed part of the letter "P" in the title, and click. The correct answers will appear on screen for each scenario. Just look to the right of the title. e.g. 321. Don't wait for a hand cursor - it won't show up. I wanted to keep this hidden from users. But it may help you if you want to go through the whole game quickly.
