I have laid out a custom survey that I want it to report both a rating scale and comments to our LMS. I am not sure what this file is missing in order to get it to report properly. I have attached a copy to the thread.
My goal is to be able to review all submissions within our lms so managers and others can run reports and review the feedback from the survey.
You have a Results slide, but you didn't connect it to the path the user follows. Since the results are submitted when the user gets to that slide, and that's how you're tracking completion, those results are never sent to the LMS.
Instead of exiting the course on your last slide, I suggest briefly taking the user to the Results slide. You can use a trigger to Exit the course when the timeline ends.
All your questions are set up as Pick One, but the Form view is blank, so Storyline doesn't know what the user is picking:
It appears you also removed the trigger to Submit the interaction, so those slides are not acting as surveys.
