Essay responses not consistently reporting in LMS

Hello,
I've been working on an essay response type module to use as a survey. I've published and tested the module in our LMS 3-4 times after publishing in the SCORM2004 3rd Edition and SCORM2004 4th Edition. The text entry fields have been set to not allow blanks or no responses.
Despite initially returning 100% of user responses (small sample size), I've tested this module with a larger group and it's "losing" answers within the LMS reporting engine. I'm unsure whether it's the LMS or the module, but I find the inconsistency most frustrating. "Lost" responses currently do not show a trend or pattern.

I'd really appreciate any insight or suggestions.

For LMS issues, we recommend first testing your course in SCORM Cloud just to get an idea on whether the issue is specific to your LMS or not.
