Adding A Table

Hey everyone,

I am building a course and need to add a table. The table has 15 columns and 5 rows. The last column is the comments section of the table which means that column will have the most words/content. If there is another option to present the information other than a table, I am open to that as well. I have attached the table, so you can see the layout of information I need to add. HELP PLEASE!
