All my users are 'passing' even though not all of them are scoring above our pass mark

We have a few courses hosted on Talent LMS, as SCORM 1.2 files. They're instructional courses with a short quiz at the end for the user to check their understanding.

I have the pass mark set to a generous 70%, but according to the reports that Talent LMS generates, all of the users who've taken the courses have passed, despite the average course score for all users being below 70%.

How can that be? What settings could I check to make sure everything is as it should be?
