Auto-start the course on the 3rd failed quiz attempt- please help!

I'm trying to build the triggers to require users to retake the course automatically if they fail the quiz 3 times. Unfortunately, it keeps restarting the quiz over and over with unlimited ...
OMG thank you so much - I have so much learning to do!! One question - what are the blue squares with the number 1? It's on all of the slides- how can I take it off?
