Best method for a learner to add information to a form in Moodle

I have a client that has a document with information to show the learner, and then they want to have a section on the document for the learner to take notes. They don't want to upload and download a document and want the learner to be able to make notes on a document and keep it all on Moodle. I have searched for the best way to do this and have looked at individual wiki's, but I am struggling to set up the document. I saw something about dataform plugin. Has anyone done anything like this for a client before with some helpful tips?
