Creating an agreement button

At the beginning of our course, we would like to include a slide that has some program stipulations listed. We would like for the student to, in effect, sign off to say that he or she agr...
I am trying to do exactly the same thing except that I need this info to be exported in the LMS. Is there a way to create this kind of button and, when the user clicks, the info appears in the LMS?
Maybe some kind of Javascript... but I don't know Java for now... So anything else that could be easier?
