What kind of form are you using? If you can create the form in Typeform or Google Forms, you can easily embed that web form in a Multimedia Embed block.
Then, you can easily retrieve learner responses from your Typeform or Google Forms account.
Would that work?
Google forms is a good idea.
That will work for internal forms, but not for the standardised government ones. It's a start though!
