Hi Lesa -  Yes, you can totally do that! Here's a quick look at how you could set that up, and the Storyline file is attached below. Keep in mind, though, that if you've included a menu on your slide, learners would still be able to use that to jump ahead to the next slide before filing in the fields, unless you've chosen restricted or locked navigation on your player. What a lot of folks do if that's a concern is just remove the menu from the player (or at least remove it from appearing on this particular slide). Hope this helps.
