Hi Amanda,
For my project, it ended up being an extension in Chrome that was the problem. It's called Dashlane and the people who created Dashlane said they're aware of the problem and are attempting to fix it. Not sure if you're using Chrome or if that issue applies to you at all, but that seems to be the cause of the bug for us.

Thanks Ken, that's good to know. I've just checked and my tester was using Safari.
