How do I set up for trainees to add their name and date of completion?

Thanks for following up! Sorry that you've hit a snag when trying to recreate the sample file I shared.
Not to worry! I'd be happy to take a closer look at the course you've built out so far. Would you be willing to share a copy of the .story file here in the discussion?
Also, it looks like part of your e-mail signature came through when you replied via e-mail. You can remove that if needed by clicking ‘Edit’ beneath your response.

After I fired off that email - I went back into it and it worked.
This is the best option for me for learners to add their name and date to the quiz results. So, thank you for all your help.
