How to graded fill in the blank and check box activity

Hello Guy's I need a help. I created the customize activity, but it cant calculate the percentage of each questions of the answer. How I create that activity?

If the user enters the values and check the box then submit of each questions.

I want if the user enters the any 3 correct answers out of 4 questions. Window will appear you have passed. 

I attached below the sample of activity.
