I got a message that the video is no longer available. If I understand this comment, I can combine survey and quiz questions in the same result slide. I usually don't do survey questions but my client requested it and I wasn't sure if it was possible.

You can have a Results Slide pull both survey and graded questions, but you won't be able to grade survey questions. I'd recommend using a Graded Results Slide so that the learner can see their score on the Results Slide. The responses to the survey questions will show up in the report generated by the LMS.
