Lock Icon Appears on Quiz Bank (Knowledge Check) When Relaunched

The course I have been developing has an ongoing issue of the lock icon re-appearing on the player menu after a quiz has already been completed. The check mark in the menu appears after quiz completion, but when the course is re-launched and resumed it is locked in the menu. In this case the quiz is a knowledge check and I'm using quiz banks to draw questions. The user is also unable to select the quiz bank from the player menu. Any advice or help with this issue would be appreciated. Thanks!
