Menu doesn't progress for Quiz questions pulled from Question Bank.

I'm pulling 20 of 58 questions from a question bank, and I'd like the menu on the left to show the user's progress. It checks off Question 1, but nothing after... What am I doing wrong? (SL360 x64) ...
