Delete the question and re-create it.
Hi Karl, thank you for the suggestion. However, deleting it and recreating it did not work. It is happening for all eight questions I created.

Can you post screenshots of:
- The Multiple Response question in Edit mode.
- The Multiple Response question in Preview mode, after the question has been correctly answered..
