Question bank - Questions marked to always be included, are always positioned at start of question draw

When I posted this course on LMS, someone pointed out the following:
1)There were no issues for taking the test the first time.
2)However, while retaking the test, it was skipping the questions. Please could you take a look? Many thanks,

I reordered the triggers and went through the test three times and was presented with all 13 questions each time.
