I've found in SL2 and SL360 that if a question bank is set to draw 1 random question... referring to the "Draw from question bank" multiple times will ALWAYS draw the same question.

I alway thought the randomisation was done when the course loads so if you have a quiz review set up you can review the correct questions, I think you would need more than one question bank to get different questions in the same instance of the course
