Quiz Reporting - Questions Not Pulling On Report

We have a course that we are using to as an activity where employees will provide information and then we'll use the report from the LMS to have a discussion with them. 

The employees responses show up in the report, but the questions do not. The questions show up as Scene1_Slide5_MultiChoice_0_0 as an example. I've also included a screenshot of the report where I was testing. 

Steps taken:

• Looked for additional settings to change back end slide names. 
• Slide names are changed to unique information, which shows in the menu. 
• Question is in the "Enter Question Here" field of the quiz slide. 
• Tracking and Reporting is set to Passed/Completed
• Publishing to SCORM 1.2

Thoughts on how to get the question to pull into the report?
