Thank you for the suggestion of using Google forms, I will look into that.
We are using SumTotal but corporately they are not allowing us to connect with external courses from other suppliers. I know the system has the capability but is not a feature enabled for us. I also know there is a feature within that system that allows for employees to upload their own course certificates but unfortunately we do not have the organizational structure in the system to make that feasible.
It is possible with Storyline and some Javascript magic.
In this sample you can upload and change the image and text. Should work on SumTotal too.
Both Image and text can be saved and read.

As is i cannot share this, as my personal login for the database it writes the content to is in it. You however can contact me at m.notermans@tilburguniversity.edu to discuss possibilities. Eg. i can create a sample you can test on your LMS, and if that works show you how to get it done. As said quite some Javascript inthere.
