It's definitely a bug that I encounter fairly often.
It's one of those bugs that only occur under certain conditions, and I've not been able to determine the exact conditions to reproduce.

It just happened to me again.
I was editing an existing quiz and added a new question at the bottom of the list and what is shown in the image below happened:
Note that the item that is overlayed over the question type choices is the last question in the list.
