The Apply to All button is useful when applying the same alt text to an asset throughout the course. To make sure I understand correctly, can you please confirm this is what you're hoping to do? Does this with a specific image, or does it appear to be random? Additionally, you'll want to make sure you're working and saving the file on your local hard drive (typically your C: drive).

yes I want to add the alt text in the media library then click apply to all so it will have the same alt text every slide in the project with that image. My project is on my c drive. It is happening at random. The alt text might also appear to be saved the next time I open it but some will eventually disappear.

Have you noticed if this happens with multiple files or only this specific project?
- If it's only this project, you can try importing your slides into a new Storyline project
- If this issue persists with multiple files, I recommend conducting a Storyline repair
