I've updated the demonstration of this approach on our website:
The demo consists of a three lesson RISE course with a short Likert style survey at the end of each lesson. We use JavaScript to store the learner responses in local browser storage and then, at the end of the course, we have a JavaScript routne that retrieves the learner responses, opens a pdf survey template, fills it out, and presents the results in a downloadable pdf file:
