Template for creating SMART Goals

I'd love to find a sample course or template on setting S.M.A.R.T. Goals. Any assistance would be appreciated!
I have a draft escape-the-room type module that is geared toward developing a SMART problem statement. I can send the source file in SL360 or PowerPoint if interested.
