Inserting a quiz and a survey in the same module

Hi there, 

Firstly, I'm new to E-Learning Heroes and want to say congrats on doing a great job!

I'm using Storyline 360 and want to have a quiz where the user will have a max of 3 attempts to pass the quiz before they have to register the score (if the user passes on first/second attempt - they can register their score at that point) , I want the quiz to show the result on completion of the quiz and then they have to go on and take a quick 2 question survey.

I looked back on an old response to this question and tried it but its not showing the actual % result - instead just showing a Pass/Fail result. Can you help, please?

Once complete I want to add to our LMS and get a report on the quiz result and the survey result

Hi Karen!

Great question! You can include both a graded question and survey question in the same results slide. If you publish for LMS using SCORM 2004, the question data will appear in the learner's report. Here's a quick 3-minute video where I've explained how to set this up in Storyline 360 and what it looks like in an LMS report.

You can definitely combine graded questions with survey questions in the same results slide. You can confirm this by adding a graded question and a survey question in a project file, and then adding a results slide. You'll see that you'll have the option to select both questions in the Quiz Settings window shown below.

Hello Jodi!

You can have a Results Slide pull both survey and graded questions, but you won't be able to grade survey questions. I'd recommend using a Graded Results Slide so that the learner can see their score on the Results Slide. The responses to the survey questions will show up in the report generated by the LMS.

Thank you for responding.  My client says the file worked perfectly and captured the survey and quiz responses in their LMS.  It was odd that it worked in SCORM 2004 but not when published to CMI5.

I noticed another issue pop up and since you were both so kind to respond I thought I'd post here.  Maybe you can help.

My client asked me to make custom buttons for moving forward and back. No problem.  I suppressed the player button and created custom ones.  But I notice when I revisit the essay and quiz slides, the built in player buttons show up upon revisiting.  This only happens on the essay and quiz questions. I did a quick screencast and attached an abbreviated version of my file here.  Any ideas on how to avoid this?  I know the answers are locked in once they are submitted and I don't expect a lot of people to go back, but I would like to know if there is away to address it.  Maybe I'm doing something wrong?
