Mandatory text entry for either SL 3 or 360

Hi friends,

I have SL 3 and 360 so info for this on either would work. I'm working on a simulation and need people to enter specific text. So, for example, there are spaces for them to add an email address, first name, and last name that I give them. Then, they have to click a specific radio button. I want them to type in all required text and then once they do that, the radio button can be clicked. When they click that, then it will go to the next slide. I do not want Next/prev buttons or any submit buttons. It should look just like they have gone to a page and these are places for them to fill out on a website. I don't know to set any of this up, especially the parts where the text has to be exactly what I'm telling them. Not sure how to trigger that the 3 places have to be filled out correctly before they can click on the Yes/radio button. Please help!!!
