Storyline quiz bug

I'm coming across a strange bug & can't find the solution. I have a storyline module which ends with a quiz. Users are supposed to do the chapters before the quiz. In my tests, if I go straight to the quiz (via the menu), my 2 question slides work correctly. But if I do the chapters as required, when I get to the 1st question slide, I select my answers, then click on SUBMIT button which appears, I then get an error message in the Inspection console. (Tests were done on articulate review platform & on our LMS.)

Thanks for your reply. I have uploaded the story file in question.
We have tried everything but just can't understand the cause of the bug and how to solve it.
