Adding a calendar to choose a date

Hi, Patricia! I thought I'd stop in to note, if you should decide to go with Michael's suggestion, unfortunately we are not able to provide support for any JS coding or modifications, but there are lots of JS gurus in the community who are more than happy to assist!
