Best practices for alt text and visibility on grouped objects?

I have searched the discussions and I can't find an answer to this question. I am retrofitting some projects in Storyline 360 to meet WCAG 2.2 AA. I was wondering about best practices when assigning alt text and visibility to grouped, interactive objects, such as knowledge checks that contain a radio button and text?

The action is tied to the group level, so do I keep visible just the group or also each object in the group? Do I assign alt text to each item in the group, or just the group?
