Sounds like you have quite a lot of issues following Sally's steps. They worked perfectly for me as in the example I share here a while back. Maybe reach out to Nejc (https://www.linkedin.com/in/nejczorgadulmin/) who offers an SL audit service as it seems you SL file needs a major overhaul.
I went back through step-by-step, and I did have a few items that needed adjusted. However, I'm still running into an issue. If the user would select some correct answer and some incorrect, the submit button will not work. The only way to get the submit button to work is if all the answers are either correct or incorrect. Does anyone know how to make it where the submit button works if they only have a few wrong? I've attached the file here so you can see that its working if all answers are correct or incorrect. I need to make the submit button work if not all the answers are correct.
