I have suggestions, however, have not a had a chance to implement them personally.
- You could use web objects and embed an online form on the course.
- you could adapt a tutorial like this one https://www.devlinpeck.com/tutorials/generate-pdf-elearning and use variables to store the text entries. Then give the learner the option to print at the end.
