Compile iterative text input into a final screen

I am trying to create a learning journal wihtin my eLearning course of 5 modules. At the end of each module the learner would note via a text input field any takeaways from that module. At the end of the course, I would like to have their entries compiled on one screen which I would then use a print to pdf javascript to allow them to download. Any ideas???

    Each text entry field you create will have a variable assigned to retain its value. At the end of the course, you can have a slide display those values by using a reference:
