Creating a Questionnaire with Checkboxes

Hello!

I am trying to create a list where there are 4 options for the participant to click on. For each item, students can pick 1 or more options. This is to be graded correct or incorrect but not for points. Do you know what kind of block I can use or how I can make this happen in Rise 360?

Item Red Purple Green Blue
Complimentary Colors x   x  
Primary Colors x     x
Rise 360

    A Knowledge Check block scores answers as correct or incorrect, but no grade is sent to the LMS. (Only a designated Quiz submits results.)

    Otherwise, for something more custom, consider a Storyline block.
