Drag and Drop Interaction showing incorrect when answers are correct

Hi there!

I'm working on a drag and drop interaction in which I have 10 shapes the user has to decide to 'include' or 'not include' by dropping them in their respective drop target.

I have reviewed and reviewed and tested and tested but I still can't seem to find out why, after I select Submit, it's showing incorrect, when I know for a fact, I selected selected all the correct answers. 

I would be happy to share my file in a secure location for someone to take a look at. 

All my settings haven't changed from any other drag and drop activity I've done in the past. So, I'm pretty stumped as to why it's doing this.
