Embedding a form into Rise

I would like a very simple form to be embedded into Rise to collect the name and email of the person at the end of the course.  Google forms look bad as it adds scrolling within scrolling. Any other suggestions?  I want to host on the web and not in our LMS because our LMS requires way too many steps for guest/outside users to get into.

    hi using firefox version 70.0.1 - found out i need to turn off 'enhanced tracking protection' - then it works :)

    You could create a fillable form using a third-party tool (like Google Forms). The form can be embedded in your lesson using a multimedia embed block. 

    Google Forms has a setting that will automatically email responses to the person filling out the form. Would that be an option for you?

  • Hi, I needed to add a form to a recent Rise project to support learners in starting a library search from inside a Rise activity, the form when submitted then passes the data to the library search website (Opening in a new window).

    The code I wrote to achieve this can be found on GitHub. The technique could be adapted to add other dynamic content types to published Articulate Rise projects.
