Form in courses

Hi,

I want to build a form inside a Rise course (SCORM 2004), where the student can fill in name, email and phone number, and post it. I want to receive a mail for every submission, but here comes the thing: I need to know the course title the form is submitted from, and it has to come automatically, since there are a lot of diferent courses, and too much job to do it manually. Any ideas?
