Learning in flow of work - experiences

I'm looking for any good examples, or war stories, from corporations who have deployed learning materials which provide (for example) always available modules that provide help, reference as well as learning.

I'm particularly interested in how modules are kept visible to staff; and even where modules might present themselves in a context-relevant way when someone is doing daily work.

I have attempted this in a small way with several clients: deploying a series of modules on their LMS as an adjunct to a classroom course, that would be available permanently afterwards as more learning and reference. Even this basic idea was tricky; the modules were presented as a series of 'tasks' that had to be ticked off by an arbitrary deadline - there didn't seem to be an alternative and it confused everyone.
