LMS status with quiz attempts (Incomplete or Failed)

I have content developed in STORYLINE 360 that has 5 lesson slides and a final quiz. The number of user quiz attempts is set at 5. My client need the follow LMS tracking:

I've tried with triggers but still have trouble.
I need to have LMS status:
- Incomplete if the quiz is not passed but quiz attempts is more than 0.
- Failed if the quiz is not passed but quiz attempts is equal to 0.
With triggers, Incomplete status is ok! But I still don't figure how to set Failed status.
I've tried JS code but problem is not solved.
