Question Slide, 2 Attempts not working

Instead of having only 2 chances per question, the try again layer shows unlimitedly.

I'm happy to help troubleshoot this issue and would like to start with clarifying questions to understand your experience better.

Does the problem happen only to specific slides? If so, what are the slide numbers concerned? I ran some tests, and the Try Again layer shows as expected, and I was only allowed two attempts.
Do you observe it when previewing or in the published output? If in the published output, is it Web, LMS, or Review 360?
