Quiz is completing even if you fail it.

Thank you for sharing your .story file!
As you mentioned, you are using Passed/Failed as your reporting option. That means that as long as the user completes the course, it will be marked as complete, but with a success status of either Passed or Failed. See below:
Changing the reporting to either Completed/Incomplete (with no success status) or Passed/Incomplete should show Incomplete when the user fails the quiz.
