You can certainly add the additional text fields through Slide View, but you won't be able to report on those "Other" text entries if they're within a question type.
Have you had the opportunity to check out Storyline? You could build something like this pretty quickly in Storyline, then report the variables that Storyline generates to an LMS for your review.
I'm curious to see if other community members can share any insight from their experiences achieving something similar to what you're looking for!

I recently updated from Storyline (3, I think) — which I used to convert PPTs to html, and which seemed to have a "quiz builder" in the past — to the Articulate 360 suite, so now I have both apps. I would love your help or a link to get me started on making this quiz in Storyline!

Hi James!
I understand you'd like some information on creating a quiz with questions that have one answer or multiple answers in Storyline 360. I'd be happy to get you some information on that! Based on the sample questions in your post, it looks like you would want to use form-based survey questions. Here's an article on Adding Form-Based Questions to help get you started.

Thanks for the links. Looking at John's links, I can see how to easily add a "pick one" or "pick many" question to my survey using Storyline. May I please call your attention to my original examples:
1. What is your favorite flavor of ice cream (pick one)?
a. Vanilla
b. Chocolate
c. Other _______ [user can type answer]2. What kinds of cars have you owned (pick all that apply)?
:: Ford
:: Chevrolet
:: Chrysler
:: BMW
:: Mercedes
:: Other _______ [user can type answer]From Luciana's links, I found documentation on how to "send the value of a variable to an LMS" but I'm still missing the middle part – how to make these question types. This is a pretty straightforward option in SurveyMonkey but I don't see it here.

I use the branching option to branch to a text entry survey slide if other is selected, this will then be stored in the LMS
