I'm sorry to hear that you ran into this issue.
We are currently tracking a known bug in Rise 360 that causes question banks to keep showing the same question in the edit field. Based on your description of the issue, you seem to be affected by this bug, so I added this community post to the report. We'll leave you an update in case we have any updates to share.
If adding new questions doesn't work, please open a case with our support team here to connect with a support engineer for further troubleshooting.

Thank you for letting me know. I tried adding new questions and same behaviour. I've contacted support and will try their suggestions - clearing cache, different browser and using private browsing to see if that helps. Is there a timeline for a fix?
